Supplementary Form

In addition to completing the CAF online application form you are asked to complete a Supplementary Information Form (SIF) to apply to St. Augustine’s School. This is your Priest reference and must be printed from this website or collected from the School Office. You take it to your Priest who completes the form for you and then you return it to St. Augustine’s where our Office Staff will copy it and return the original to you.

On the advice of the Education Commission of the Archdiocese of Southwark the governing bodies of the Catholic primary schools in Lewisham have agreed the following statement in order to clarify how its faith-based oversubscription criteria will be interpreted whilst churches are closed or attendance at church is not possible due to COVID19. This statement has been shared widely with parishes in order to ensure that determinations made by parish priests concerning regular practice of the faith are being made on a consistent basis:
